Pharmacology

Heparin inhibits reactions that lead to the clotting of blood and the formation of fibrin clots both in vitro and in vivo. Heparin acts at multiple sites in the normal coagulation system. Small amounts of heparin in combination with antithrombin III (heparin cofactor) can inhibit thrombosis by inactivating activated Factor X and inhibiting the conversion of prothrombin to thrombin. Once active thrombosis has developed, larger amounts of heparin can inhibit further coagulation by inactivating thrombin and preventing the conversion of fibrinogen to fibrin. Heparin also prevents the formation of a stable fibrin clot in inhibiting the activation of the fibrin stabilizing factor.

Pharmacokinetics:

Heparin is not absorbed from the gastrointestinal tract.After intravenous or subcutaneous injection heparin is extensively bound to plasma proteins.It does not cross the placenta and it is not distributed into breast milk.The hlf-life of heparin depends on the dose and route of administration as well as the method of calculation and is subject to wide inter- and-intra individualvariation; a range of 1 to 6 hours with an average of 1.5 hours has been cited.It may be slightly prolonged in renal impairment,decreased in patients with pulmonary embolism, and either increased or decreased in patients with liver disorders.Heparin is taken up by the mainly as metabolites, although after large doses up to 50%may be excreted unchanged.

Indications:

Atrial fibrillation with embolization:
Treatment of acute and chronic consumption coagulopathies (disseminated intravascular coagulation);
Prevention of clotting in arterial and heart surgery;
Anticoagulant therapy in prophylaxis and treatment of venous thrombosis and its extension;
(In a low-dose regimen) for prevention of postoperative deep venous thrombosis and pulmonary embolism in patients undergoing major abdomino-thoracic surgery or who for other reasons are at risk of developing thromboembolic disease;
Prophylaxis and treatment of pulmonary embolism;
Prophylaxis and treatment of peripheral arterial embolism.

Contraindications:

Due to the potential toxicity of benzyl alcohol in neonates, solutions containing benzyl alcohol must not be used in this patient population. Parenteral preparations with benzyl alcohol should not be used for fluid or sodium chloride replacement. Heparin sodium should not be used in patients: With hypersensitivity to heparin; With an uncontrollable active bleeding state , except when this is due to disseminated intravascular coagulation; With inability to perform suitable blood-coagulation tests, e.g., whole-blood clotting time, partial thromboplastin time, etc. at required intervals. There is usually no need to monitor effect of low-dose heparin in patients with normal coagulation parameters.

Precautions:

Do not use unless solution is clear and container is undamaged. Use aseptic technique for single or multiple entry and withdrawal from all containers. Since heparin sodium in this product is derived from animal tissue, it should be used with caution in patients with a history of allergy.

• ibuprofen ;
• indomethacin ;
• nicotine cigarettes, gum, lozenges, or skin patches;

• cold, allergy, or sleep medications ;
• salicylates such as aspirin.

Side Effects:

Heparin can cause you to have bleeding episodes while you are using it and for several weeks after you stop: nosebleed, blood in your urine or stools, black or tarry stools, or any bleeding that will not stop. Allergic reaction to heparin: nausea, vomiting, sweating, hives, itching, trouble breathing, swelling of your face, lips, tongue, or throat, or feeling like you might pass out.
Some people receiving a heparin injection have had a reaction to the infusion (when the medicine is injected into the vein): nauseated, light-headed, sweaty, or short of breath during or after a heparin injection.

• sudden numbness or weakness, especially on one side of the body;
• sudden severe headache, confusion, problems with vision, speech, or balance;
• chest pain, sudden cough, wheezing, rapid breathing, fast heart rate;
• pain, swelling, warmth, or redness in one or both legs;

Heparin inhibits reactions that lead to the clotting of blood and the formation of fibrin clots both in vitro and in vivo. Heparin acts at multiple sites in the normal coagulation system. Small amounts of heparin in combination with antithrombin III (heparin cofactor) can inhibit thrombosis by inactivating activated Factor X and inhibiting the conversion of prothrombin to thrombin. Once active thrombosis has developed, larger amounts of heparin can inhibit further coagulation by inactivating thrombin and preventing the conversion of fibrinogen to fibrin. Heparin also prevents the formation of a stable fibrin clot in inhibiting the activation of the fibrin stabilizing factor.

Heparin inhibits reactions that lead to the clotting of blood and the formation of fibrin clots both in vitro and in vivo. Heparin acts at multiple sites in the normal coagulation system. Small amounts of heparin in combination with antithrombin III (heparin cofactor) can inhibit thrombosis by inactivating activated Factor X and inhibiting the conversion of prothrombin to thrombin. Once active thrombosis has developed, larger amounts of heparin can inhibit further coagulation by inactivating thrombin and preventing the conversion of fibrinogen to fibrin. Heparin also prevents the formation of a stable fibrin clot in inhibiting the activation of the fibrin stabilizing factor.

Pharmacokinetics: 

Heparin is not absorbed from the gastrointestinal tract.After intravenous or subcutaneous injection heparin is extensively bound to plasma proteins.It does not cross the placenta and it is not distributed into breast milk.The hlf-life of heparin depends on the dose and route of administration as well as the method of calculation and is subject to wide inter- and-intra individualvariation; a range of 1 to 6 hours with an average of 1.5 hours has been cited.It may be slightly prolonged in renal impairment,decreased in patients with pulmonary embolism, and either increased or decreased in patients with liver disorders.Heparin is taken up by the mainly as metabolites, although after large doses up to 50%may be excreted unchanged.

Indications: 

Atrial fibrillation with embolization:
Treatment of acute and chronic consumption coagulopathies (disseminated intravascular coagulation);
Prevention of clotting in arterial and heart surgery;
Anticoagulant therapy in prophylaxis and treatment of venous thrombosis and its extension;
(In a low-dose regimen) for prevention of postoperative deep venous thrombosis and pulmonary embolism in patients undergoing major abdomino-thoracic surgery or who for other reasons are at risk of developing thromboembolic disease;
Prophylaxis and treatment of pulmonary embolism;
Prophylaxis and treatment of peripheral arterial embolism.

There is usually no need to monitor effect of low-dose heparin in patients with normal coagulation parameters.

Precautions: 

Do not use unless solution is clear and container is undamaged. Use aseptic technique for single or multiple entry and withdrawal from all containers. Since heparin sodium in this product is derived from animal tissue, it should be used with caution in patients with a history of allergy.

Drug Interactions: 

• another blood thinner, such as warfarin ;
• digoxin ;
• dipyridamole ;
• hydroxychloroquine ;
• ibuprofen ;
• indomethacin ;
• nicotine cigarettes, gum, lozenges, or skin patches;
• nitroglycerin ;
• an antibiotic such as demeclocycline , doxycycline , minocycline , or tetracycline ;
• cold, allergy, or sleep medications ;
• salicylates such as aspirin.

Side Effects: 

Heparin can cause you to have bleeding episodes while you are using it and for several weeks after you stop: nosebleed, blood in your urine or stools, black or tarry stools, or any bleeding that will not stop. Allergic reaction to heparin: nausea, vomiting, sweating, hives, itching, trouble breathing, swelling of your face, lips, tongue, or throat, or feeling like you might pass out.
Some people receiving a heparin injection have had a reaction to the infusion (when the medicine is injected into the vein): nauseated, light-headed, sweaty, or short of breath during or after a heparin injection.

• sudden numbness or weakness, especially on one side of the body;
• sudden severe headache, confusion, problems with vision, speech, or balance;
• chest pain, sudden cough, wheezing, rapid breathing, fast heart rate;
• pain, swelling, warmth, or redness in one or both legs;
• trouble breathing;
• (in an infant) extreme drowsiness, weakness, or gasping for breath; or
• fever, chills, runny nose, or watery eyes.

Less serious heparin side effects may include:
• mild pain, redness, warmth, or skin changes where the heparin was injected;
• mild itching of feet; or
• bluish-colored skin.

• Store at 20 to 25 C°
• Protect from light and freezing
